Phoenix Place, located at 725 Washburn Way, provides three distinct levels of care for adults (over 18 years of age) with mental health needs. We helps individuals live in the least restrictive environment possible and assist them in regaining self-reliance and control over their own lives by providing a structured day program and access to community activities. The goal is to assist patients to move to more independent living situations when ready.
Residential Program:
The program provides nine residential beds for clients who have been living in the Oregon State Hospital or other more restrictive environments. Residents require skilled medical services or long-term care because they have a prolonged acute mental illness and are unable to perform their activities of daily living in a home setting. The focus of this program is to teach independent living skills to enable residents to become as independent as possible.
Crisis Respite Program:
Phoenix Place has five beds for people who are experiencing a mental health crisis. These individuals are not in need of hospitalization, but need supervised care and respite in order to focus on their situation and find solutions to their problems. Residents can remain in the unit for up to 30 days.
Secure Care Non-Hospital Unit:
This program opened 12/1/2000 to provide a safe environment in which to assess the needs of the most acutely mentally ill adults. Based on that assessment, some individuals are transferred to a state psychiatric facility, while others stabilize during their stay in the secure care area. Then, as soon as they are able, they can move on to a less restrictive environment to complete the stabilization process.
